• Date of your Last Dental Visit
• Ask if you have questions, concerns, or special requests
• We will coordinate an appointment time
• Direct you to our Hippa Safe New Patient forms where you will provide insurance, medical and dental history
• A Covid-19 Wellness Form will be sent to you

Day of Your Visit

• You will be screened upon arrival.
